Your Early Supported Discharge Program (ESD) Rehab Team
The ESD rehab team
Your ESD rehab team may include: physiotherapist; occupational therapist; speech therapist; social worker; nurse; special care councillor; dietician as needed. Additionally, the ESD coordinator and a case manager will arrange your therapy visits and facilitate at-home rehabilitation needs. The service we offer is specific to you and is dependent on rehabilitation goals established with you. This ESD team provides specialized stroke interventions at home promoting independence and quality life after stroke.
Your ESD Team comprises:
Physiotherapists (PT): The PT helps you improve your physical function. Emphasis will be on transfers, walking, and taking stairs, for you to become as independent as possible with your mobility
Occupational Therapist (OT): The OT works with you to regain or improve your functional independence with your daily activities. This consists of the activities you do every day, e.g. bathing, dressing, using the toilet, meal preparation and other homemaking activities
Speech-Language Pathologist (SLP): The SLP specializes in the assessment and treatment of speech, language, swallowing and cognitive-communication disorders
Special Care Councilors (SCC): The SCC in collaboration with the rehab professionals aims to help with your continued learning /practice of new functional skills and assisting you to achieve your rehab goals
Nurse: Your nurse is responsible for your health care needs and is supportive of your well-being throughout your participation in ESD program
